Activate your Neighbourhood Family Event Application

Activate Your Neighbouhood (AYN)- Family Events are your chance to be part of introducing active play and healthy eating into your neighbourhood. If you have an idea to get children 0-12 and their families out, active and having fun in your neighbourhood, our Healthy Kids staff at the City want to help you make it happen. Thank you for submitting your idea. Please fill in this form with as much info as possible to help us make your event a success.

Activate Your Neighbourhood is an initiative of the City of Greater Sudbury, in partnership with the Healthy Kids Community Challenge of Greater Sudbury. By submitting your request form you agree to the requirements of Activate Your Neighbourhood Events:
 

  • Events must be free and open to all families in your community.
  • Water must be served as the main beverage at your event.
  • The Gift Card must be used to purchase vegetables and/or fruits for your event.
  • The organizer must be at least 18 years old and remain on site during the event.
  • A signed application form and simple event report-back form must be submitted by the organizer.
  • All participating children under 10 must be accompanied by an adult responsible for their supervision.
  • Approval of events and support is dependent on available funding..
  • Activities must support physical activity and healthy lifestyles.

The information you submit will be used to help the Healhy Kids Community Challenge (City of Greater Sudbury) promote and support your event. Your personal information (not including the Event Contact Information provided) will not be shared publicly. By submitting the form you give consent for City of Greater Sudbury staff to contact you and to send you further information about the Healthy Kids Community Challenge.
